The Mafia is a relatively recent arrival in Withmore City. Offshoot syndicate interests from Neo-York established themselves by taking over several businesses and criminal enterprises in San Mayo.

Notionally an incorporated organization with a council seat, the Mafia continues to oversee and control illegal operations. Its public standing has not severed its criminal roots. Like the other major syndicates, it combines business interests with participation in the city's underworld.

San Mayo's streets are largely claimed by the Sinners, while the Mafia holds influence over the neighborhood's organized crime and criminal enterprises. Its position is part of the broader competition among syndicates over Red's businesses and territory, where arrangements can change and disputes extend beyond visible street rivalries.
